These exams, also known as pre-participation physical exams or PPEs, are an opportunity for healthcare providers to discuss health issues with children and their parents or guardians.
Parents Please fill out your child's Preparticipation Physical Evaluation-Medical History page before your appointment.
Our Provider will conduct a physical examination (if applicable, please bring your child's eye glasses or contact lenses for a visual acuity test"
